This Public Health Nurse focuses on direct client care, contact investigations, health education and treatment support for people affected by TB in different public settings.

This temporary full time role is across North York and Scarborough and it is hybrid with Monday to Friday hours that add up to 35 hours every week. In this role, nurses do home visits, offer clinical support and help manage cases for people who need it throughout Toronto.

Job Description

  • Public Health Nurse supports tuberculosis prevention, case management and client care services across Toronto Public Health.
  • This is a full time temporary role with 6 month and 9 month vacancies available.
  • The position follows a hybrid work structure based in North York and Scarborough.
  • Staff work Monday to Friday with 7 hour shifts totaling 35 hours each week.
  • The role includes outreach work, clinical care, home visits and public health education services.
  • Nurses work closely with healthcare teams and public agencies in a supportive healthcare setting.

Responsibilities

  • Provide public health nursing services for individuals, families and groups.
  • Conduct TB case management and contact investigations.
  • Complete home visits, outreach activities and clinical assessments.
  • Review client health needs and prepare care plans and referrals.
  • Keep up electronic health records and their paperwork with digital tools.
  • Offer counseling, treatment support for clients and also their families.
  • Gather samples and then arrange lab testing.
  • Review laboratory reports, chest x rays and treatment follow ups with clients and doctors.
  • Support health promotion programs, workshops and public education activities.
  • Work with schools, shelters, workplaces and public agencies to address health concerns.
  • Prepare reports, presentations and program related documentation.
  • Support emergency public health response activities when needed.

Requirements

  • A bachelor’s degree in nursing from a recognized university and valid Registered Nurse license from the College of Nurses of Ontario are required.
  • Work background in public health nursing or communicable disease prevention and control is required.
  • Work background with outreach services, home visits or direct patient care within multidisciplinary teams is required.
  • Work background as a Registered Nurse supporting vulnerable clients facing housing, substance use or mental health challenges is required.
  • Must hold a valid Ontario Class G driver’s license with regular access to a vehicle and can drive within Toronto.
  • Strong knowledge of tuberculosis prevention, disease control and epidemiological practices is required.
  • Skilled in using electronic medical records, computerized systems and complex health databases including IPHIS.
  • Can prepare reports, health education materials, flyers and public information documents.
  • Can work in different hours including evenings, weekends, overtime and shift work during public health emergencies.
  • Speaking another language will be considered an asset.
  • Excellent interaction skills and can maintain positive public relations with clients and partners.
  • Can work independently in a self directed manner.
  • Strong knowledge of municipal, provincial and federal legislation connected to communicable disease reporting and infection control is required.
