Job description

  • Conduct psychosocial assessments including mental health and social support needs
  • Develop and update care plans for residents
  • You must provide counseling and therapeutic support
  • Facilitate group sessions and support programs
  • Respond to crises including mental health and conflict situations
  • Advocate for residents’ rights and support decision-making processes
  • Support families with care planning and legal or ethical concerns
  • Work with healthcare teams to deliver coordinated care
  • Lead or support programs that improve resident well-being
  • Prepare reports, case notes and documentation
  • Build partnerships with community resources and services
  • Assist discharge planning and care transition processes

Job requirements
