Job description

Conduct psychosocial assessments including mental health and social support needs
Develop and update care plans for residents
You must provide counseling and therapeutic support
Facilitate group sessions and support programs
Respond to crises including mental health and conflict situations
Advocate for residents’ rights and support decision-making processes
Support families with care planning and legal or ethical concerns
Work with healthcare teams to deliver coordinated care
Lead or support programs that improve resident well-being
Prepare reports, case notes and documentation
Build partnerships with community resources and services
Assist discharge planning and care transition processes
Staff and team members receive training and guidance from the trainers

Job requirements

A Bachelor’s degree in social work
The applicant must be registered with the Ontario College of Social Workers and Social Service Workers in good standing
Background working with adults facing complex health or mental health challenges
The candidate has the ability to provide counseling services while connecting residents with available resources.
The candidate shows expertise in understanding human behavior and implementing different methods of intervention.
Strong interaction skills with residents, families and care teams
Can manage sensitive situations with professionalism and discretion
Knowledge of long term care regulations and healthcare systems
Ability to manage workload and maintain accurate records
Master of social work or gerontological education is considered an asset
