Job Description
- Candidates working in this post are appointed to monitor adherence to treatment plans and response to health care interventions.
- They’re also required at this post to liaise with community care providers to obtain health histories and to facilitate transitional care at discharge for both remand and intermittent inmates who applied for this post.
- Candidates working in this post are appointed to participate in case management.
- People must be an advocate for and encourage patient participation in the plan of care.
- Candidates are also required to act as a resource for patients, staff and external stakeholders by sharing their knowledge and expertise.
- People who applied for this post are required to work collaboratively as members of the inter-professional team to develop, implement and evaluate patient-specific plans of care applied for this post.
Job requirement
- Candidates must be registered as Registered Nurses with the College of Nurses of Ontario and be in good standing to be considered as an asset for this post.
- They must be able to provide full scope of services to patients with complex healthcare needs, and/or unpredictable outcomes.
- Candidates must provide emergency care, acute care, crisis intervention, and health teaching.
- They’re also required at this post for the counselling to patients with routine or complex health care needs, and/or unpredictable outcomes.
