Job Description
- Candidates will have to work as a team member of the priority Home Service.
- Candidates will also have to collaborate with the interpersonal hospital team.
- Candidates will have to ensure the candidate coordinated with the programs.
- Candidates will also initiate the referral and the service required to deliver professional and non-professional services.
Job requirement
- Candidates must attain experience as a case coordinator in community health services.
- Candidates must attain good computer knowledge and computer operating skills.
- Candidates must attain interpersonal skills, oral and written communication skills.
- Candidates must attain demonstrated leadership abilities.
Work setting
- The availability of a driving license is a must for being eligible for this post.
