They coordinate the delivery of a broad range of professional and non-professional services based on the Case Coordinator Guidelines. They act as a liaison and maintain communication with agencies and facilities involved with the client group.

Job description
- Candidates have to develop a plan of care for Continuing Care Program including statements of client need, objectives, service provision and evaluation criteria.
- Candidates will maintain current case count: ensure proper submission of statistics.
- They perform assessment and prioritization of patterns/needs.
- Candidates have to establish an appropriate discipline coordinator.
- They are responsible for implementing and coordinating care plans, which may include initiating medical rehabilitation and consultative services as necessary to meet client needs.
Job requirement
- Candidates must have two years directly related community experience preferred.
- They must have previous experience as a Case Coordinator in Community Health Services preferred.
- They must have experience with EFT implementation considered as asset.
- Candidates must have knowledge and experience with computers.
- They must have effective oral and written communication skills.
Work setting
- Candidates must attain BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD or related health/human service degree required.
- They must have a valid driver’s license and a vehicle.
- Candidates must have licensure/registration as per professional designation required.
