Job Description
- Candidates working in this post are appointed to conduct focused health assessments using and adapting assessment tools and techniques based on resident needs.
- They’re also appointed to complete health history into the resident’s situation, including physical, psychosocial, emotional, cultural and ethnic dimensions of health.
- Candidates working in this post are appointed to perform physical examinations and identify and interpret normal and abnormal findings.
- Candidates working in this post are appointed to order diagnostic investigations and interpret results using evidence-based clinical reasoning.
- They’re also appointed to ensure Accreditation Canada standards are evident in daily practice.
- They’re required to ensure resident files and health care records are properly maintained in compliance with applicable legislation and divisional expectations.
- They’re also appointed to identify and collaborate with the Medical Director on, education working at this post.
Job requirement
- They must have a current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class and be entitled to practice.
- They must attain dedicated experience as a Nurse Practitioner in long-term care, acute care, rehabilitation, continuing care or primary care.
- They must have experience in long-term care, and providing empathetic care for the elderly and those living in long-term care to be eligible for this post.
- Candidates must have dedicated certification in Geriatric Nursing from the Canadian Nursing Association or equivalent experience.
- Candidates must attain the ability to exercise independent judgment and assess situations and problems efficiently and effectively.
- Candidates must attain the ability to foster effective working relationships to be eligible for this post.

- Candidates must attain leadership skills, working well within an interprofessional team.
