Job Description
- Candidates working in this post are required to conduct mental health assessments for triage and ongoing care applied for this post.
- They’re required at this post to plan and implement treatment plans.
- People applying for this post must be able to monitor adherence and response to treatment plans and healthcare interventions.
- People applying for this post must liaise with community care providers to obtain mental health histories and facilitate transitional care at discharge to be eligible for this post.
- People who applied for this post must have led case management by initiating patient-specific plans of care.
Job requirement
- Candidates must be registered as Registered Nurses with the College of Nurses of Ontario and be in good standing to be eligible for this post.
- People must work collaboratively with others, such as the inter-professional team, correctional staff and community agencies to be eligible for this post.
